LeafEase Terms and Conditions
1. Important Information and Who We Are
These Terms of Service (“Terms”) govern the use of the LeafEase website (“Site”) which are operated by Guardia Health Ltd (“we”, “us”, “our”), a company registered in England and Wales (Company No. 15695083), located at: Suite 5a, 2nd Floor, 1–9 Castle Street, Hinckley, LE10 1DA, United Kingdom.
LeafEase provides digital tools to help patients manage their health journey, easier, cheaper, quicker and securely share relevant data with authorised healthcare partners, including clinicians, research organisations, and regulated medical technology providers.
By using our services, you agree to these Terms and our Privacy Policy (which forms part of these Terms). We may update these Terms periodically; significant changes will be communicated to you, and continued use of our services constitutes acceptance of any changes.
2. Consent for Medical Assessment & Healthcare Services
By providing your explicit consent, you authorise LeafEase to:
- Access, review, and process your medical information to find the right treatment provider for you.
- Provide medical and healthcare services necessary for your assessment, including GP consultation (if required) and related clinical procedures.
- Share relevant information with your GP or other healthcare professionals when necessary for your clinical safety or continuity of care.
- Share information with safeguarding authorities, if required for your protection or the protection of others.
Your data will be lawfully processed under GDPR for medical diagnosis and the provision of healthcare.

A confidential clinical record will be retained for eight years following your last contact, in accordance with UK medical recordkeeping requirements.
3. Registration and Eligibility
3.1 Eligibility Requirements
To register with LeafEase, you must:
- Be at least 18 years old.
- Provide accurate and up-to-date contact and personal information.
- Use the Site/App in accordance with these Terms.
- Consent to necessary information being shared with clinical partners to ensure safe prescribing and dispensing.
3.2 Registration Process
You may register using the online form. You are responsible for maintaining the accuracy of your information and for all activity carried out.
4. Account Security
You must create a secure password and keep it confidential.
If you suspect unauthorised access, notify us immediately at info@leafease.co.uk.
Any actions performed under your account will be treated as your own.
5. Telehealth – Risks and Limitations
By using our services, if a Clinician appointment is required, you understand and accept that:
- Technical limitations (e.g., poor image or audio quality) may affect assessment quality.
- Connectivity issues may cause delays.
- Incomplete or inaccurate medical history may impact clinical decisions.
- Although safeguards are in place, digital transmission of data carries inherent risks.
6. Acceptable Use
You must NOT:
- Use the Site/App for unlawful, fraudulent, or harmful purposes.
- Upload misleading, offensive, or inappropriate content.
- Attempt to hack, bypass security, or misuse the platform.
- Copy, distribute, reverse-engineer, or commercially exploit any part of the Site/App.
All content, software, trademarks, and branding belong to LeafEase or its licensors. You may use materials only for personal, non-commercial purposes.
7. Medical Disclaimer
- LeafEase does not replace professional medical advice or provide general medical diagnosis outside the assessment process.
- A doctor–patient relationship is established only during consultation with a registered clinician.
- Always seek emergency medical care when needed. For urgent concerns, contact 999 or NHS 111.
8. Treatment Terms
8.1 Scope of Care
LeafEase provides clinical assessment and treatment exclusively for medical cannabis
We do not provide general medical, crisis, or emergency services.
8.2 Exclusive CBMP Care
You agree:
- Not to obtain CBMP prescriptions from other providers while under LeafEase’s care, unless you supply a formal discharge summary.
- That clinicians may decline to prescribe or continue treatment based on clinical judgment.
8.3 Medication Use and Risks
You understand that:
- CBMPs are unlicensed “specials” medicines and may carry risks, including side effects, contraindications, and withdrawal symptoms.
- You must take medication exactly as prescribed and must not smoke, share, sell, or divert prescribed cannabis.
- If adverse effects occur, you must stop using the medication and seek clinical advice promptly.
8.4 Dispensing and Pharmacy Regulations
- Medication costs are separate from clinic fees.
- Medication is dispensed by regulated pharmacies on a named-patient basis.
8.5 Treatment Decisions
A clinical assessment does not guarantee a prescription.
If you decline the clinician’s recommended treatment plan, you acknowledge that service fees remain non-refundable once the consultation has occurred.
9. Your Responsibilities as a Patient
You agree to:
- Provide complete, accurate, and truthful medical and personal information.
- Complete all required pre-consultation questionnaires
- Attend all scheduled appointments (fees may apply for missed appointments).
- Inform LeafEase promptly of any changes to your health, medication, or circumstances.
- Follow clinical advice and medication instructions.
10. Fees, Payments & Cancellations
10.1 Payment
- All clinic services must be paid in advance by debit card or credit card via the link that will be sent to you.
- Fees are payable in GBP (£).
- Medication fees (if required) are separate from the cost of any consultations.
10.2 Cancellations
- Consultations must be cancelled at least 72 hours in advance to avoid charges.
- Failure to complete pre-consultation questionnaires will result in automatic cancellation, and rebooking fees may apply.
- Please note that failure to complete the pre-consultation questionnaire and upload the required Summary Care Record and identification documents will result in automatic cancellation of the appointment, and rebooking fees may apply.
10.3 Refunds
Refunds are only issued in exceptional circumstances, at LeafEase’s discretion.
Once a clinical service has been provided, fees are non-refundable.
11. Prescription Dispensing
You consent to LeafEase sharing necessary identifying information with partner pharmacies to safely dispense CBMP prescriptions.
12. Multidisciplinary Care
Your care may involve regulated professionals such as doctors, nurses, and pharmacists. Each clinician is responsible for their own professional conduct and regulatory compliance. You consent to LeafEase securely sharing relevant medical information with authorized Multidisciplinary Team (MDT) members for the purpose of providing and coordinating your healthcare.
13. Summary Care Record (SCR) – Consent for Medical Information
LdeafEase may request access to your Summary Care Record (SCR) or equivalent medical history from your registered GP, with your explicit consent.
You agree that:
- Your SCR may be accessed and reviewed by qualified LeafEase clinicians solely for clinical assessment and procedure.
- Information received from your GP will be processed securely in line with UK data protection law and our Privacy Policy.
- You may request a copy of any GP-provided information at any time.
14. Liability
We do not exclude liability for:
- Death or personal injury caused by our negligence.
- Fraud or fraudulent misrepresentation.
- Any liability that cannot be excluded under UK law.
Otherwise:
- Our liability is limited to the total fees paid by you in the 12 months preceding any claim.
- We are not liable for indirect, incidental, special, or business losses, or for advice provided by independent clinicians.
15. Declaration & Consent
By using LeafEase’s services, you confirm that:
- You consent to LeafEase requesting and accessing your SCR or equivalent medical records.
- You consent to your personal and health data being used and shared as described in these Terms and our Privacy Policy.
- You understand the responsibilities, risks, and limitations associated with medical cannabis treatment.
- You are not aware of any reason that would prevent you from giving valid consent.
- You understand your right to request copies of your medical information held by LeafEase.
- You have read and agree to LeafEase’s Terms & Conditions and Privacy Policy.
